A physician’s take: how to improve the clinical documentation process.

As physicians, we make it our priority to provide quality care with excellent continuity to our patients.

The clinical documentation process can be one of the most challenging parts of practicing family medicine. Recording complete and accurate information from a patient encounter has significant impacts on the quality of care we can provide. It's also essential from a legal point of view to have records that accurately depict patient encounters.

It's common in family practices to become overwhelmed with the volume of patients. This usually leads to clinical documentation being pushed to the back burner. We typically try to catch up on this process after hours or on the weekends. Besides the risk of burning out, leaving our charting process for our free time can lead to loss of important details which should be recorded in a patient's chart.

Did you know: More than one in four physicians and residents reported high levels of burnout according to a 2017 CMA National Physician Health Survey.

How opting for 3M™ M*Modal Fluency Direct software helped the clinical documentation process.

As we look to optimize our time in a clinic, having a front-end speech recognition software allows physicians to complete clinical documentation as they go. It's quicker than the traditional process of typing out notes.

After implementing the 3M™ M*Modal Fluency Direct software at my clinic, here are the benefits I saw in my clinical documentation process.

1. Create accurate records quicker.

I find that I can make notes of patient encounters quicker after each consultation. This process helps me to free up my time.

2. Increased accuracy.

My accuracy is much higher if I complete the notes immediately after I meet with a patient. The software's speech recognition capabilities help me complete my notes quicker, which motivates me to come to this process immediately after a consultation. With better retention of my discussion with a patient, my documentation accuracy has seen an increase.

3. Easy implementation of the software.

Once 3M™ M*Modal Fluency Direct software was loaded onto my computer, I attended a training session. The session itself was straightforward, simple, and user-friendly, and I could easily implement the software into my practice.

Did you know: 3M™ M*Modal Fluency Direct software comes with a cloud-based, artificial intelligence (AI) powered solution that leverages our speech understanding software for a contextual understanding of the physician narrative, to drive accuracy.

Free up time to focus on our patients.

The 3M™ M*Modal Fluency Direct software has been a wonderful addition to my medical practice.

It has allowed me to create more useful, accurate, and thorough patient encounter notes quicker than before. This gives me more time to focus on spending time with my patients and has also improved my work-life balance.

I would encourage every physician to consider adopting the software in their practice.
